  • Scheduling appointments

    Prior to your initial appointment, please fill out your new client forms at least 24 hours prior to your appointment. New client forms will be emailed to you through your Client Portal once your appointment is confirmed.

  • payments

    Total cost of appointment is due at the time of service. An active credit card must be kept on file.

  • timeliness

    I reserve the right to cancel your appointment if you are more than 10 minutes late. Appointment sessions will end on time.

  • refill requests

    Medications will be refilled at each appointment, if it is clinically appropriate, so that you will not run out before your next appointment. Refill prescriptions will be called in for current patients who maintain their regularly scheduled appointments. Refill requests will have a response time within 24-48 business hours. Emergency refill requests are not available. Prescriptions will not be called in after operating business hours or during holidays. No new prescriptions will be called in without first having an appointment. Prescriptions will not be called in for anyone who repeatedly misses their appointments.

  • cancellation policy

    If you miss your appointment, you will be charged 50% of the cash pay rate for the appointment. To ensure you do not get charged this fee, please cancel your appointment 24 hours, or earlier, ahead of your appointment start time.

  • Client Portal

    After you have completed a pre-screen questionnaire and offered an appointment, you will be sent a link to my secure, online client portal. There, you can schedule or cancel follow-up appointments, request refills, report mild to moderate symptoms or side effects, and provide non-urgent medical correspondence. Messaging should not be used for emergencies.

  • controlled medicines

    Prescriptions for controlled substances will not be refilled in between appointments. These medicines will also not be refilled if appointments are missed. This includes benzodiazepines and medications for ADHD.

    More than one benzodiazepine is contraindicated and will not be prescribed.

    Controlled medications will not be called into pharmacies that are out of state.

    Lost prescriptions will not be replaced. You will need to wait until your next scheduled refill.

    Chronic use of benzodiazepines is also contraindicated. If prescribed benzodiazepine, a formalized plan will be in place to discuss time frame of use and treatment options for non-controlled medications will be considered.

    If you are prescribed a controlled substance, you will be required to complete a random mandatory drug screen at least twice per year.

    If medication abuse or misuse is suspected, I reserve the right to cancel your prescription.
